Zat\u00edmco tehdy byla pova\u017eov\u00e1na za vz\u00e1cnou, dnes u&nbsp;n\u00e1s podle odhadu \u010cesk\u00e9 alzheimerovsk\u00e9 spole\u010dnosti \u017eije s&nbsp;demenc\u00ed, jej\u00ed\u017e hlavn\u00ed p\u0159\u00ed\u010dinou je pr\u00e1v\u011b Alzheimerova nemoc, 171 tis\u00edc lid\u00ed, co\u017e odpov\u00edd\u00e1 po\u010dtu obyvatel Plzn\u011b. Jak\u00e9 jsou p\u0159\u00ed\u010diny a&nbsp;prvn\u00ed varovn\u00e9 sign\u00e1ly t\u00e9to nemoci? Jak se j\u00ed lze br\u00e1nit a&nbsp;kde naj\u00edt na Sedmi\u010dce pomoc?<\/p>\r\n<p>[\/perex]<\/p>\r\n<p>Lidsk\u00fd mozek obsahuje v\u00edce ne\u017e 100 miliard nervov\u00fdch bun\u011bk, kter\u00e9 spolu komunikuj\u00ed pomoc\u00ed speci\u00e1ln\u00edch spojen\u00ed zvan\u00fdch synapse. Tyto nervov\u00e9 bu\u0148ky, neboli neurony, neust\u00e1le p\u0159en\u00e1\u0161ej\u00ed informace a\u00a0sign\u00e1ly. U\u00a0Alzheimerovy nemoci jsou jako prvn\u00ed posti\u017eeny pr\u00e1v\u011b synapse, co\u017e znamen\u00e1, \u017ee neurony nemohou spr\u00e1vn\u011b komunikovat. To vede k\u00a0probl\u00e9m\u016fm s\u00a0pam\u011bt\u00ed a\u00a0my\u0161len\u00edm.<\/p>\r\n<p>P\u0159esn\u00e1 p\u0159\u00ed\u010dina Alzheimerovy nemoci nen\u00ed zcela zn\u00e1m\u00e1, ale v\u011bdci se domn\u00edvaj\u00ed, \u017ee hlavn\u00ed roli hraj\u00ed dv\u011b \u0161kodliv\u00e9 l\u00e1tky: amyloidn\u00ed plaky a\u00a0tau klubka. Amyloidn\u00ed plaky jsou tvrd\u00e9 shluky protein\u016f, kter\u00e9 se hromad\u00ed mezi nervov\u00fdmi bu\u0148kami a\u00a0naru\u0161uj\u00ed jejich komunikaci. Tau klubka jsou zamotan\u00e9 proteiny uvnit\u0159 neuron\u016f, kter\u00e9 po\u0161kozuj\u00ed jejich strukturu. Tyto l\u00e1tky zp\u016fsobuj\u00ed, \u017ee neurony odum\u00edraj\u00ed, co\u017e vede k\u00a0postupn\u00e9 ztr\u00e1t\u011b mozkov\u00fdch funkc\u00ed.<\/p>\r\n<p><strong>Osobnost se m\u011bn\u00ed<\/strong><br \/>\r\nAlzheimerova nemoc naru\u0161uje \u010d\u00e1sti mozku, co\u017e vede k&nbsp;poklesu kognitivn\u00edch funkc\u00ed, jako jsou my\u0161len\u00ed, pam\u011b\u0165 a&nbsp;\u00fasudek, nebo exekutivn\u00edch funkc\u00ed, je\u017e zahrnuj\u00ed schopnost \u010dlov\u011bka organizovat sv\u00e9 my\u0161lenky a&nbsp;\u010dinnosti. Nemoc za\u010d\u00edn\u00e1 postupn\u011b, kdy \u010dlov\u011bk m\u00e1 pot\u00ed\u017ee se zapamatov\u00e1n\u00edm ned\u00e1vn\u00fdch ud\u00e1lost\u00ed, zat\u00edmco vzpom\u00ednky na star\u0161\u00ed ud\u00e1losti z\u016fst\u00e1vaj\u00ed neporu\u0161en\u00e9. Postupem \u010dasu se objevuj\u00ed probl\u00e9my s&nbsp;jazykem a&nbsp;komunikac\u00ed, zmatenost a&nbsp;dezorientovanost. N\u00e1sledn\u011b maj\u00ed pacienti pot\u00ed\u017ee s\u00a0vykon\u00e1v\u00e1n\u00edm b\u011b\u017en\u00fdch denn\u00edch aktivit, jako je j\u00eddlo, obl\u00e9k\u00e1n\u00ed a\u00a0hygiena. Rychlost v\u00fdvoje nemoci se u\u00a0ka\u017ed\u00e9ho li\u0161\u00ed. Jak nemoc postupuje, lid\u00e9 \u017eij\u00edc\u00ed s\u00a0demenc\u00ed maj\u00ed st\u00e1le v\u011bt\u0161\u00ed probl\u00e9my s\u00a0vyjad\u0159ov\u00e1n\u00edm a\u00a0rozhodov\u00e1n\u00edm, nedokon\u010duj\u00ed my\u0161lenky a\u00a0jsou \u010d\u00edm d\u00e1l v\u00edce zmaten\u00ed. Osobnost takov\u00e9ho \u010dlov\u011bka se m\u011bn\u00ed a\u00a0v\u00a0posledn\u00edch stadi\u00edch nemoci u\u017e nen\u00ed schopen se o\u00a0sebe s\u00e1m postarat. \u201eV\u00a0d\u016fsledku nemoci se m\u016f\u017ee st\u00e1t, \u017ee mil\u00e1 star\u0161\u00ed d\u00e1ma s\u00a0prvorepublikovou v\u00fdchovou za\u010dne pou\u017e\u00edvat sprost\u00e1 slova, a\u010dkoliv jsme mysleli, \u017ee je v\u016fbec nezn\u00e1. D\u011bde\u010dek, kter\u00fd zbo\u017e\u0148oval d\u011bti, se bude najednou v\u00a0jejich p\u0159\u00edtomnosti c\u00edtit \u0161patn\u011b a\u00a0bude to d\u00e1vat najevo. Spole\u010densk\u00fd \u010dlov\u011bk, kter\u00fd m\u011bl v\u017edycky r\u00e1d z\u00e1bavu, se v\u00a0d\u016fsledku nemoci bude uzav\u00edrat do sv\u00e9ho sv\u011bta. \u010clov\u011bk, kter\u00fd r\u00e1d v\u0161e organizoval, najednou nev\u00ed, jak na to,\u201c \u0159\u00edk\u00e1 Martina M\u00e1tlov\u00e1, \u0159editelka \u010cesk\u00e9 alzheimerovsk\u00e9 spole\u010dnosti (\u010cALS).<\/p>\r\n<p><strong>Demence nen\u00ed st\u00e1rnut\u00ed<\/strong><br \/>\r\nV\u00a0souvislosti s\u00a0Alzheimerovou nemoc\u00ed se \u010dasto setk\u00e1v\u00e1me s\u00a0pojmem demence. Jak\u00fd je mezi nimi rozd\u00edl? Demence je stav zp\u016fsoben\u00fd z\u00e1va\u017en\u00fdm po\u0161kozen\u00edm mozku, nap\u0159\u00edklad v\u00a0d\u016fsledku traumatu nebo otravy, kter\u00fd postihuje hlavn\u011b mozkovou k\u016fru. Nej\u010dast\u011bj\u0161\u00ed p\u0159\u00ed\u010dinou demence je pr\u00e1v\u011b Alzheimerova nemoc, zat\u00edmco druhou nej\u010dast\u011bj\u0161\u00ed p\u0159\u00ed\u010dinou jsou poruchy c\u00e9vn\u00edho z\u00e1soben\u00ed mozku. Demence nen\u00ed b\u011b\u017enou sou\u010d\u00e1st\u00ed st\u00e1rnut\u00ed. \u201eJedn\u00e1 se o\u00a0chorobn\u00fd stav. Podobn\u011b jako ochabuj\u00ed svaly, ve st\u00e1\u0159\u00ed ochabuje i\u00a0mozek. Demence je ale n\u011bco jin\u00e9ho. A\u00a0a\u010dkoliv je v\u011bk jedn\u00edm z\u00a0hlavn\u00edch rizikov\u00fdch faktor\u016f vzniku demence, je t\u0159eba v\u011bd\u011bt, \u017ee \u010dty\u0159i z\u00a0p\u011bti lid\u00ed star\u0161\u00edch 80 let demenc\u00ed netrp\u00ed,\u201c dod\u00e1v\u00e1 Martina M\u00e1tlov\u00e1.<\/p>\r\n<p>Poruchy pam\u011bti zp\u016fsobuj\u00edc\u00ed \u010dlov\u011bku obt\u00ed\u017ee v\u00a0ka\u017edodenn\u00edm \u017eivot\u011b jsou \u010dasto patrn\u00e9 sp\u00ed\u0161e jeho bl\u00edzk\u00fdm ne\u017e jemu samotn\u00e9mu. Nemocn\u00ed tyto probl\u00e9my mnohdy p\u0159i\u010d\u00edtaj\u00ed nadm\u011brn\u00e9mu stresu, \u00fanav\u011b nebo vy\u0161\u0161\u00edmu v\u011bku. Poruch\u00e1m pam\u011bti je v\u0161ak t\u0159eba v\u011bnovat pozornost. I\u00a0takzvan\u00e1 m\u00edrn\u00e1 kognitivn\u00ed porucha se m\u016f\u017ee v\u00a0p\u0159ibli\u017en\u011b 50&nbsp;% p\u0159\u00edpad\u016f postupn\u011b vyvinout do n\u011bkter\u00e9 z\u00a0forem demence. Proto je d\u016fle\u017eit\u00e9 na jak\u00e9koli zm\u011bny pam\u011bti v\u010das reagovat.<br \/>\r\n<br \/>\r\n<\/p>\r\n<p><a href=\"https:\/\/www.praha7.cz\/kdyz-pamet-mizi\/weed-1\/\" rel=\"attachment wp-att-78431\"><img loading=\"lazy\" src=\"https:\/\/www.praha7.cz\/wp-content\/uploads\/2024\/08\/weed-1.png\" alt=\"\" width=\"548\" height=\"742\" class=\"alignnone wp-image-78431 size-full\" srcset=\"https:\/\/www.praha7.cz\/wp-content\/uploads\/2024\/08\/weed-1.png 548w, https:\/\/www.praha7.cz\/wp-content\/uploads\/2024\/08\/weed-1-295x400.png 295w\" sizes=\"(max-width: 548px) 100vw, 548px\" \/><\/a><br \/>\r\n<br \/>\r\n<\/p>\r\n<p><strong>P\u0159\u00ed\u010diny a\u00a0prevence<\/strong><br \/>\r\nV\u011bdci si st\u00e1le nejsou jisti p\u0159\u00ed\u010dinami Alzheimerovy nemoci. V\u00a0posledn\u00edch n\u011bkolika letech je velk\u00e1 pozornost v\u011bnov\u00e1na rizikov\u00fdm faktor\u016fm demence, kter\u00e9 je mo\u017en\u00e9 ovlivnit zm\u011bnou chov\u00e1n\u00ed \u010di \u017eivotn\u00edho stylu. Podle aktu\u00e1ln\u00ed zpr\u00e1vy komise \u010dasopisu Lancet je t\u011bchto rizikov\u00fdch faktor\u016f 14 a\u00a0pravd\u011bpodobn\u011b stoj\u00ed a\u017e za 45&nbsp;% v\u0161ech p\u0159\u00edpad\u016f demence. Seznam rizikov\u00fdch faktor\u016f najdete v\u00a0jednom z\u00a0box\u016f.<\/p>\r\n<p>Riziko rozvoje Alzheimerovy nemoci v\u00fdrazn\u011b roste po 65.&nbsp;roce \u017eivota. Alzheimerovou nemoc\u00ed v\u0161ak mohou onemocn\u011bt i\u00a0mlad\u0161\u00ed lid\u00e9. V\u00a0jejich p\u0159\u00edpad\u011b bude s\u00a0velkou pravd\u011bpodobnost\u00ed hr\u00e1t roli d\u011bdi\u010dnost.<\/p>\r\n<p>Dal\u0161\u00edm faktorem je pohlav\u00ed. Demence postihuje v\u00edce \u017eeny ne\u017e mu\u017ee. Podle v\u00fdro\u010dn\u00ed zpr\u00e1vy \u010cesk\u00e9 alzheimerovsk\u00e9 spole\u010dnosti je aktu\u00e1ln\u00ed po\u010det \u017een s\u00a0demenc\u00ed v\u00edce ne\u017e 116 tis\u00edc, zat\u00edmco u\u00a0mu\u017e\u016f se toto \u010d\u00edslo pohybuje kolem 54 tis\u00edc. Tento rozd\u00edl p\u0159itom nejsp\u00ed\u0161 nem\u016f\u017ee b\u00fdt vysv\u011btlen t\u00edm, \u017ee \u017eeny maj\u00ed vy\u0161\u0161\u00ed pr\u016fm\u011brn\u00fd v\u011bk do\u017eit\u00ed, pravd\u011bpodobn\u011b zde hraj\u00ed roli i\u00a0dal\u0161\u00ed soci\u00e1ln\u00ed a\u00a0biologick\u00e9 faktory.<\/p>\r\n<p>Absence zdrav\u00e9ho \u017eivotn\u00edho stylu zahrnuj\u00edc\u00edho kvalitn\u00ed stravov\u00e1n\u00ed, dostatek pohybu, ale i\u00a0du\u0161evn\u00ed pohodu a\u00a0aktivn\u00ed spole\u010densk\u00fd \u017eivot je rizikov\u00fdm faktorem v\u011bt\u0161iny onemocn\u011bn\u00ed, a\u00a0ani Alzheimerova nemoc nen\u00ed v\u00fdjimkou. V\u0161echny vyjmenovan\u00e9 sou\u010d\u00e1sti optim\u00e1ln\u00edho \u017eivotn\u00edho stylu jsou kl\u00ed\u010dov\u00fdmi slo\u017ekami prevence Alzheimerovy nemoci.<\/p>\r\n<p>Uveden\u00e9 rizikov\u00e9 faktory spolu vz\u00e1jemn\u011b souvisej\u00ed a\u00a0navz\u00e1jem se ovliv\u0148uj\u00ed. Du\u0161evn\u00ed nepohoda, nap\u0159\u00edklad v\u00a0podob\u011b deprese, vede k\u00a0poruch\u00e1m sp\u00e1nku a\u00a0p\u0159\u00edjmu potravy, zv\u00fd\u0161en\u00e9 \u00fanav\u011b, ztr\u00e1t\u011b z\u00e1jmu o\u00a0cokoli a\u00a0sn\u00ed\u017een\u00ed aktivity. Dal\u0161\u00edm aspektem ovliv\u0148uj\u00edc\u00edm du\u0161evn\u00ed pohodu je tak\u00e9 osam\u011blost, kter\u00e1 je v\u00a0sou\u010dasn\u00e9 dob\u011b velk\u00fdm probl\u00e9mem hlavn\u011b u\u00a0lid\u00ed \u017eij\u00edc\u00edch ve m\u011bstech. \u201eZtr\u00e1ta z\u00e1jmu o\u00a0v\u011bci kolem negativn\u011b ovliv\u0148uje pam\u011b\u0165 \u2013 schopnost v\u0161t\u00edpen\u00ed informace a\u00a0jej\u00edho n\u00e1sledn\u00e9ho vybaven\u00ed. Nel\u00e9\u010den\u00e1 deprese se u\u00a0osob star\u0161\u00edho v\u011bku m\u016f\u017ee projevovat stejn\u011b jako demence a\u00a0m\u016f\u017ee spolup\u016fsobit jako spou\u0161t\u011bc\u00ed mechanismus oslabov\u00e1n\u00ed kognitivn\u00edch funkc\u00ed. Demence se stejn\u011b jako deprese statisticky v\u00fdznamn\u011b m\u00e9n\u011b vyskytuje u\u00a0lid\u00ed, kte\u0159\u00ed vyv\u00edjej\u00ed pravidelnou fyzickou aktivitu a\u00a0vedou aktivn\u00ed spole\u010densk\u00fd \u017eivot,\u201c dod\u00e1v\u00e1 Iva Kol\u00e1\u0159ov\u00e1 z\u00a0Centra pro studium dlouhov\u011bkosti a\u00a0dlouhodob\u00e9 p\u00e9\u010de p\u0159i Fakult\u011b humanitn\u00edch studi\u00ed Univerzity Karlovy.<\/p>\r\n<p>Pr\u00e1v\u011b osam\u011blost, du\u0161evn\u00ed pohoda a\u00a0kvalita \u017eivota ve m\u011bstech jsou souvislosti, kter\u00e9 zkoum\u00e1 evropsk\u00fd projekt Recetas, jeho\u017e c\u00edlem je p\u0159in\u00e9st d\u016fkazy ve prosp\u011bch nefarmakologick\u00fdch a\u00a0preventivn\u00edch p\u0159\u00edstup\u016f v\u00a0p\u00e9\u010di o\u00a0ve\u0159ejn\u00e9 zdrav\u00ed, zejm\u00e9na pak v\u00a0seniorsk\u00e9 populaci, kter\u00e1 je podle opakovan\u00fdch zji\u0161t\u011bn\u00ed nejv\u00edce ohro\u017eena omezen\u00edm \u010di ztr\u00e1tou spole\u010densk\u00fdch kontakt\u016f. Projekt tak\u00e9 zkoum\u00e1 vliv pohybu v\u00a0m\u011bstsk\u00e9 zeleni na zdrav\u00ed a\u00a0speci\u00e1ln\u011b na tvorbu nov\u00fdch p\u0159\u00e1telstv\u00ed a\u00a0spole\u010densk\u00fdch kontakt\u016f. Recetas nyn\u00ed zve nov\u00e9 \u00fa\u010dastn\u00edky do studie. Projekt je ur\u010den osob\u00e1m star\u0161\u00edm 65 let, v\u00edce informac\u00ed z\u00edsk\u00e1te na str\u00e1nce <a href=\"http:\/\/www.cello-ilc.org\/recetas\">www.cello-ilc.org\/recetas<\/a> nebo na telefonn\u00edm \u010d\u00edsle 776\u00a0630&nbsp;431.<\/p>\r\n<p><strong>V\u010dasn\u00e1 diagn\u00f3za<br \/>\r\n<\/strong>Diagnostika Alzheimerovy nemoci je komplexn\u00ed proces, kter\u00fd vy\u017eaduje kombinaci r\u016fzn\u00fdch metod a\u00a0p\u0159\u00edstup\u016f. Bohu\u017eel neexistuje jedin\u00fd test, jen\u017e by mohl jednozna\u010dn\u011b potvrdit jej\u00ed diagn\u00f3zu. Tu m\u016f\u017ee u\u010dinit pouze l\u00e9ka\u0159 na z\u00e1klad\u011b pe\u010dliv\u00e9ho zhodnocen\u00ed \u0159ady faktor\u016f, nejen osobn\u00ed a\u00a0rodinn\u00e9 anamn\u00e9zy, ale p\u0159edev\u0161\u00edm vyhodnocen\u00ed zm\u011bn kognitivn\u00edch funkc\u00ed, pam\u011bti a\u00a0chov\u00e1n\u00ed pacienta v\u00a0posledn\u00ed dob\u011b. D\u016fle\u017eitou roli hraj\u00ed tak\u00e9 krevn\u00ed testy a\u00a0zobrazovac\u00ed metody. Lid\u00e9 star\u0161\u00ed 65 let (a\u00a0mlad\u0161\u00ed 80 let) by m\u011bli u\u00a0sv\u00fdch praktik\u016f podstoupit screeningov\u00e9 vy\u0161et\u0159en\u00ed v\u00a0r\u00e1mci preventivn\u00ed prohl\u00eddky ka\u017ed\u00e9 dva roky.<\/p>\r\n<p>Pam\u011b\u0165 si m\u016f\u017eete nechat vy\u0161et\u0159it tak\u00e9 na kontaktn\u00edch m\u00edstech \u010cesk\u00e9 alzheimerovsk\u00e9 spole\u010dnosti, v\u00a0Praze je takov\u00fdm m\u00edstem s\u00eddlo \u010cALS v\u00a0ulici \u0160im\u016fnkova 1600\/5 v\u00a0Kobylis\u00edch. Pam\u011b\u0165 se vy\u0161et\u0159uje pomoc\u00ed sady test\u016f a\u00a0vy\u0161et\u0159en\u00ed je zdarma. V\u00fdsledky testu uk\u00e1\u017eou, zda jsou pacientovy probl\u00e9my s\u00a0pam\u011bt\u00ed m\u011b\u0159iteln\u00e9 a\u00a0zda by m\u011bl d\u00e1le nav\u0161t\u00edvit odborn\u00e9ho l\u00e9ka\u0159e (neurologa, psychiatra, anebo geriatra), kter\u00fd pomoc\u00ed dal\u0161\u00edch test\u016f zjist\u00ed, co je p\u0159\u00ed\u010dinou poruchy pam\u011bti, a\u00a0p\u0159\u00edpadn\u011b zah\u00e1j\u00ed vhodnou l\u00e9\u010dbu. P\u0159\u00ed\u010din poruch pam\u011bti je cel\u00e1 \u0159ada, nemus\u00ed za nimi st\u00e1t jenom Alzheimerova nemoc.<\/p>\r\n<p><strong>Nad\u011bje na dosah<\/strong><br \/>\r\nSou\u010dasn\u00e1 medic\u00edna sice neum\u00ed Alzheimerovu chorobu vyl\u00e9\u010dit, um\u00ed ji ale l\u00e9\u010dit. M\u00e1me k\u00a0dispozici l\u00e9ky, kter\u00e9 zm\u00edrn\u00ed n\u011bkter\u00e9 p\u0159\u00edznaky nemoci, do ur\u010dit\u00e9 m\u00edry zlep\u0161\u00ed i\u00a0kognitivn\u00ed funkce. V\u00fdznamn\u00e9 jsou tak\u00e9 nefarmakologick\u00e9 metody a\u00a0p\u0159\u00edstupy a\u00a0zejm\u00e9na prevence, kter\u00e1 m\u016f\u017ee zabr\u00e1nit a\u017e t\u0159etin\u011b p\u0159\u00edpad\u016f demence. \u201eKrom\u011b prevence jsou aktu\u00e1ln\u011b velk\u00e9 nad\u011bje spjaty s\u00a0takzvanou antiamyloidovou l\u00e9\u010dbou. L\u00e9k, kter\u00e9 dok\u00e1\u017ee rozb\u00edt amyloidov\u00e9 plaky, je schv\u00e1len v\u00a0n\u011bkolika zem\u00edch v\u010detn\u011b USA anebo Japonska. Evropsk\u00e1 agentura pro l\u00e9\u010div\u00e9 p\u0159\u00edpravky ale vzhledem k\u00a0pom\u011brn\u011b v\u00fdznamn\u00fdm vedlej\u0161\u00edm \u00fa\u010dink\u016fm a\u00a0podle jej\u00edho n\u00e1zoru tak\u00e9 nedostate\u010dn\u00e9mu efektu jeho u\u017e\u00edv\u00e1n\u00ed v\u00a0r\u00e1mci EU nepovolila. Jin\u00fd l\u00e9k s\u00a0obdobn\u00fdm \u00fa\u010dinkem proch\u00e1z\u00ed schvalov\u00e1n\u00edm v\u00a0USA,\u201c \u0159\u00edk\u00e1 Martina M\u00e1tlov\u00e1 z\u00a0\u010cALS. Pr\u00e1v\u011b \u010cesk\u00e1 alzheimerovsk\u00e1 spole\u010dnost na leto\u0161n\u00ed podzim p\u0159ipravuje n\u011bkolik tradi\u010dn\u00edch akc\u00ed. V\u011bt\u0161inou se uk\u00e1\u017ee, \u017ee podobn\u00e9 probl\u00e9my \u0159e\u0161\u00ed i\u00a0jin\u00e9 slu\u017eby po cel\u00e9 Praze. Sd\u00edlen\u00edm konkr\u00e9tn\u00edch p\u0159\u00edb\u011bh\u016f se p\u0159ed\u00e1v\u00e1 dobr\u00e1 praxe.
